Referral Process

 

 

Once the reffereal form has been completed  we will interview the applicant  with their support worker, health proffesional , family member or friend in attendance. If applicant is succesful a move in date will be set.

Interview Process

 

 

The interview process is designed to identify current support needed for applicant and a comprehensive risk assesment. Applicants are required to provide ID.
A holistic person-centred approach tailored to each individual’s needs and rights in relation to age, gender, race, belief, culture, sexuality, and disability is created.

We use the Outcomes Star to deliver person-centred (tailored) support plans. This is a model recognised by social and health care professionals which measures and supports change and enables positive outcomes.
To  empower  the supported adult to realise their true potential and become a powerful contributor to society.
The duty of care will apply to the supported adult at all times. Supported adult  must  actively work with staff to fully access the support on offer.
It is compulsory for all supported adults  to engage in employment, education or training. The scheme has a close working relationship with local colleges, schools and external training providers, referrals from partner agencies.
There is a waiting list system in operation.
